懷孕期婦女與緊急二尖瓣膜氣球整型術之應用：病例報告與文獻回顧

摘要

心臟二尖瓣膜狹窄在處於生育期婦女身上可能引發致命的病症，嚴重者包括會發生心臟衰竭併肺高壓、肺水腫及呼吸衰竭，甚至造成孕婦及胎兒死亡。目前妊娠期的病患已能安全的接受二尖瓣膜氣球整型術，且效果良好，但假如臨床診斷與心導管介入處理過慢，產婦及胎兒仍會產生無可避免的併發症。在此，我們將報告一位懷孕病患，因過去風濕性心臟病引發嚴重二尖瓣狹窄，因妊娠併發急性心肺衰竭而緊急接受瓣膜氣球整型術之病例。

Balloon mitral valvuloplasty has been performed safely during pregnancy with good results, but maternal complications and influent fetal outcome during pregnancy still exist. Prognostic factors of rheumatic mitral stenosis during pregnancy and puerperium were the mitral valve area and functional class strongly associated with maternal complications but not associated with fetal/neonatal events. Here, we present a patient who suffered from severe mitral stenosis complicated by severe pulmonary hypertension, acute pulmonary edema, acute respiratory failure, and cardiogenic shock, and the patient improved dramatically after emergent balloon mitral valvuloplasty.
