本研究以彰化基督教醫院為研究對象,分析其證據醫學的推行狀況,其推行/使用證據醫學的考量因素;並進一步探討證據醫學如何解決知識流通障礙、以及如何對知識管理價值鏈產生貢獻等。本研究以個案研究方法為主,並利用深度訪談進行資料蒐集,以內容分析法進行資料分析。 研究結果發現:1.證據醫學對個人知識發展的幫助最大;2.推行證據醫學的障礙之一在於醫師時間不足;3.證據醫學可提供良好的溝通機制;4.證據醫學的推行可促進知識管理價值鏈活動之進行更為順暢。 針對欲推行證據醫學的醫療單位,本研究提出以下建言:1.證據醫學的推行須有高階管理者的支持;2.完整教育訓練計畫,可使醫師們清楚瞭解證據醫學意涵;3.完善資訊系統可讓醫師於存取知識時更為容易;4.適當的獎勵與懲罰可激勵醫師投入證據醫學的領域。
This study tries to explore and analyze the factors associated with the implementation of evidence-based medicine (EBM) in Changhua Christian Hospital. Besides, how EBM has overcome knowledge flow barriers and contributed to knowledge management chain in an organization were also discussed. A case study was used to accomplish these research purposes. Data were collected by interviews and content analysis was performed for analyzing data. The study results firstly show that individuals rather than the organization benefit most from EBM in terms of knowledge development. Second, one of the most critical barriers regarding the implementatin of EBM is the lack of sufficient time for physicians. Third, EBM is believed to be an ideal communication mechanism for physicians. Fourth, EBM facilitates the activities in knowledge management value chain. Based upon the findings, we provide some suggestions for hotspital managers who would like to introduce EBM in their hospitals. Firstly and the most importantly, EBM needs total support from the top management. Secondly, well-designed educational training programs help phsycians to understand the spirits and benefits of EBM. Thirdly, information systems make it much easier for physicians when retrieving knowledge. Fourthly, reward systems and appropriate panel policies may motivate physicians to commit themselves to EBM.
