目的:以物聯網(Internet of Things, IoT)概念將醫療資訊系統(Hospital Information Systems, HIS)與智慧手機及住院查房行動載具(iPAD)進行介接,期利用智慧科技工具提升住院病人會診時效。方法:將iPAD 與HIS 進行介接,再利用全Wi-Fi 環境開發會診通知以簡訊發送至醫師智慧手機,iPAD 同步啟動電子病歷資料庫,呈現照護所需資訊,再利用iPAD完成會診回覆。結果:(一) 推動前(2014 年3 月∼8 月)會診個案8,342 件,平均每件完成時效為19.08 小時;推動半年後(2014 年9 月∼2015 年2 月)個案11,615 筆,平均每件時效為12.67 小時,提升會診時效達6.41 小時。(二) 一般會診24 小時內完成率由69.7%,提升至81.9%。結論:智慧化應用需要創新整合模式,建議醫院管理者引進新科技產品,透由使用者的經驗與巧思,以物聯網方式創造更多加值功能。
Objectives: In this study the Internet of things (IoT) concept was applied to connect hospital information systems, smart mobile phones, and mobile devices for hospital rounds (iPads) to improve the timeliness of patient consultation. Methods: Using the existing wireless fiber hospital environment, an inpatient consultation notification system, which transmitted instant messages to the public smart mobile phones of consultant physicians, was developed. In addition, the personal iPads of physicians were synchronized with the electronic patient record database and displayed information required for caregiving, such as patient consultation lists, reasons for consultation, and electronic patient records. After patient consultation, the iPads were used to compile consultation responses. Results: (1) Before the application of IoT in this study (March–August 2014), the number of clinical consultation cases was 8,342 and the average completion time of each case was 19.08 hours. Six months after the application (September 2014–February 2015), the total number of consultation cases was 11,615 and the average completion time of each case was 12.67 hours. Thus, the consultation time efficiency improved by 6.41 hours. (2) At baseline, the rate of finishing a consultation within 24 hours increased from 69.7% to 81.9%. Conclusions: Smart application requires innovative integration models. Through user experience and ingenuity, hospital supervisors are encouraged to expand the functions of newly introduced technological products using the concept of IoT to create added value.
