Objectives: Leadership support and positive attitudes are prerequisites for safe performance; therefore, effective leadership behaviors are essential in building a positive attitude and a culture ofoutstanding organizational safety. Positive attitudes and a culture of safety are also decisive factors in safe performance. The aims of this study were to investigate the relationships among leadership style, attitudes about safety, and safe performance, and to determine if there was a mediating effect of attitudes about safety between leadership style and safe performance. Methods: This was a cross-sectional study, and a probability sample of subjects was surveyed with a structured questionnaire. Subjects were medical personnel from a medical center in eastern Taiwan. Data were analyzed with descriptive statistics, inferential statistics, and structural equation models. Results: In the mediation analysis, attitude about safety was significantly positively associated with safe performance (path coefficient=0.46, p<0.001), but leadership style had no significant association with safe performance (path coefficient=0.01, p>0.05). Conclusions: A positive attitude about safety promotes a safe hospital environment, and improves the overall quality of healthcare.
