利用谷歌地圖呈現醫院層級間三高處方用藥重複率改善結果的比較

摘要

目的：比較不同層級醫院三高藥品重複用藥日數比率之改善情形。方法：擷取2001年第3季至2016年第3季（共25季）衛生福利部中央健康保險署網路公佈各醫院的三高（即高血壓、高血糖及高血脂）給藥日數重複率。整理出完整三高指標資料的有效樣本各408、414及359家醫院。利用量表信度及效度指標：(1)評估三高指標資料的品質；(2)依據谷歌地圖製作醫院三高的圖示儀表板；(3)加權層級醫院三高指標資料的改善成果分數，評定哪層級醫院表現最優？利用Kendall係數（W）評量三高指標績效排名評分之一致性。結果：結果顯示，(1)各醫院三高指標資料的建構效度品質不如其信度品質，主因地區醫院三高資料的不穩定；(2)谷歌地圖能夠清楚且容易呈現圖示儀表板；(3)三高給藥日數重複率的改善成果，醫學中心與區域醫院不相上下且優於地區醫院與基層診所。Kendall's W=0.91，說明三高指標績效排名符合其一致性（卡方=8.23，自由度=3，p=.04）。結論：本研究利用谷歌地圖呈現三高藥品重複用藥日數比率的成果儀表板，對於衛生福利部中央健康保險署網站醫療品質資訊公開之其他42項指標，提供一具網路科技應用的可行性示範。

Objectives: To evaluate the improvement of therapeutic duplication (TD) of medication in Taiwan hospitals for outpatients with high blood pressure, high blood sugar, and high blood lipids, and shown them on Google Maps. Methods: Data from the 3rd quarter of 2001 to the 3rd quarter of 2016 of TD rates of the mentioned drugs were downloaded from the website. A total of 359 hospitals were included. We applied scale quality indicators to examine data quality and select the best in performance improvement on TD drugs across types of hospital. Results: We found that: (1) Data quality was ensured; (2) dashboards on Google maps were to interpret the improvements of TD drugs; (3) both medical centers and regional hospitals earn the merit of performance improvement in the three-type TD drugs. Conclusions: The achievement with dashboards on Google Maps inspires other indicators that can be compared for hospitals in the future.
