Evaluation of Quality Performance of Hospitals in Taiwan by Using Composite Medical Quality Indicators

以多面向醫療品質指標整合評估台灣醫院品質績效

摘要


Objectives: This study aims to analyze the hospital quality performance for different hospital accreditations and divisional business groups according to the NHI Healthcare Quality Information Disclosure Network. Methods: This study attempted to use three comprehensive medical quality indicators called overall quality, medication quality and non-medication quality based on factor analysis of public quality reporting of 212 hospitals from 2016-2018 disclosed by Taiwan's Healthcare Quality Information Disclosure Network. Results: The empirical results show that the overall quality and non-medication quality are stable, while the medication quality is improving steadily. In terms of the hospital levels, medical centers have the highest quality performance. There is no statistically significant difference in the quality of the six divisional business groups. However, we still need to pay attention to the medical quality performance in the eastern district and the offshore islands. Conclusions: This study effectively simplifies the quality indicator items and provides a thorough understanding of the overall medical quality performance in Taiwan. Our research suggests that the government provide additional budget subsidies to district hospitals, the eastern district, and the offshore islands to improve their medical quality of care.

目的:本研究旨在以多面向整合評估台灣醫院品質績效,並進而比較不同評鑑等級與不同分區業務組之品質績效。方法:本研究利用衛福部健保署公布之全民健康保險醫療品質資訊公開網,針對「醫院總額指標」進行分析,並利用因素分析法整合建構出「整體品質」、「用藥品質」及「非用藥品質」三項綜合醫療品質指標,藉以評估2016年至2018年212家醫院之醫療品質表現。結果:本實證結果顯示整體品質與非用藥品質表現大致穩定,而用藥品質表現越來越佳。醫院層級別的品質表現皆以醫學中心較佳,而分區業務組的品質表現雖無顯著差異,不過仍需多加留意東區業務組與離島醫院的醫療品質表現。結論:本研究有效地簡化多面向醫院品質指標,以全面了解台灣醫院整體醫療品質之表現。我們建議政府能提供地區醫院、東區業務組與離島醫院額外預算之補助,以協助其改善醫療服務品質。
