以科技接受模式探討影響醫療人員鼓勵病人使用健康存摺意願之功能性因素

Using Technology Acceptance Model to Identify Functions of "My Health Bank" that May Critically Improve Healthcare Providers' Willingness of Recommendation

摘要


目的:「健康存摺」普及率偏低,本文藉「科技接受模式」探討影響醫療人員推薦病人使用「健康存摺」意願的系統功能因素。方法:樣本為1,315位醫療人員,含醫師(12.7%)、藥師(5.97%)、與護理師(81.4%)。利用探索性因素分析決定系統構面,複迴歸分析評估功能的有用性與易用性對醫療人員願意推薦病人使用健康存摺意願的影響。結果:系統功能的五個外部構面皆對自覺易用性有顯著影響(P<0.01),然而「門診與處置」、「申請與登入」對自覺有用性沒有顯著影響(P=0.30、P=0.09)。自覺易用性、有用性正向顯著影響醫療人員推薦病人使用意願(P<0.001)。結構方程分析結果則支持「自覺易用性」與「自覺有用性」均為顯著的中介因子,且「自覺有用性」的中介效果明顯高於「自覺易用性」的中介效果。結論:存摺系統功能中與有用性相關的面向,是醫療人員決定是否推薦給病人使用的重要因素。不過,醫療人員也擔心診斷資料的揭露可能威脅自身的自主權。

Objectives: "My Health Bank (MHB)," a web-based cross-platform developed by Taiwan Minister of Health and Welfare (MOHW), has a low penetration rate nowadays. In this research, we used "Technology Acceptance Model" to identify the critical functions in MHB that can improve healthcare providers' willingness to recommend this website to patients. Methods: A total of 1,315 self-administered questionnaire were completed by physicians (12.7%), pharmacists (5.9%), and nurses (81.4%) nurses, respectively. Exploratory factor analysis was used to determine function domains of MHB; and general multiple linear regression model was performed to assess associations of perceived usefulness (PU) and perceived ease-of-use (PEOU) with healthcare providers' willingness to recommend the use of MHB. Results: Five function-related domains were significantly associated with PEOU (P<0.01). However, "general and medical treatment information in out-patient clinic" and "software registry and log-in," were not significantly associated with PU (P=0.03, P=0.09). Both PEOU and PU may positively and significantly increase healthcare providers' willingness to recommend MHB (P<0.001). Structural Equation Modeling analysis supported the notation that both PEOU and PU were significant mediators, with a greater mediation effect noted for PEOU. Conclusions: The functions, especially PU of MHB, are critical factors that may increase healthcare providers' willingness to encourage patients adopting MHB as a health management tool. However, the revealing of detailed information to patients also raised concerns about healthcare providers' autonomy.
