目的:利用平衡計分卡探討醫療機構因應癌症篩檢政策之策略,及運作阻礙的問題。方法:研究對象為南區某醫療體系之四家不同層級醫院執行癌篩醫療人員,研究期間為2017年1到6月,進行6場焦點訪談,共17位受訪者同意並簽署受試者同意書後,進行全程錄音。訪談紀錄經繕打成逐字稿,透過直接內容分析進行關鍵論題分析。結果:在平衡計分卡的策略架構下,本研究充分歸納受訪者訪談內容中的關鍵論題,包括組織領導與支持態度、衛生政策因素與經費挹注、內部流程改善配套、員工激勵與訓練措施,以及民眾推廣策略等。結論:管理者可利用平衡計分卡了解醫療院所內部組織運作與遇到執行障礙問題,以充分提升癌篩成效。
Objectives: A balanced scorecard was used to investigate strategies and barriers when health organizations implement national cancer screening policy. Methods: This study recruited health professionals who were responsible for cancer screening services and/or promotion activities in four hospitals (one medical center, two regional hospitals and one local hospital) under the health system in the southern Taiwan. Six focus group interviews were conducted during January to June 2017 with total 17 subjects from those four hospitals agreed to participate and sign inform consent. The interview was recorded and transcribed. Direct content analysis was used to explore themes from the transcripts. Results: Based on the interview responses from the participating health professionals, this study generalized several key themes following the balanced scorecard, which included organizational leadership and substantial help from top leaders, governmental funding support and health policy adjustment, doing practices for improvement internal process, improving staff motivation and training, and cancer screening health promotion strategies. Conclusions: Through a focus group qualitative research, this study provided more understanding regarding with how healthcare institutions operated and provided cancer screening services internally and encountered difficulties. Therefore, this study may provide implications for hospital managers and policy makers regarding the implementation of strategies and effective allocation health resources for national cancer screening policy.
