Here we present pseudothrombocytopenia (PTCP) of blood samples in the presence of ethylenediaminetetraacetic acid (EDTA) in a patient with systemic lupus erythematosus. Fluctuating low platelet counts not correlated with the clinical presentation were found in routine complete blood counting on an automated cell counter. Platelet clumps were identified on peripheral blood smears. PTCP in this patient was recognized to occur rapidly in the blood samples anticoagulated with EDTA and was found to be time and temperature-dependent. Moreover, the phenomenon of PTCP was not reversible, irrespective of temperature, once the platelet clumping had taken place. Although PTCP is devoid of pathological significance, identification of PTCP is important and of clinical significance to avoid unnecessary investigations and medical management on spurious thrombocytopenia.
