自體周邊血液幹細胞之收集與冷凍保存及應用於臨床自體周邊血液幹細胞移植:彰化基督教醫院經驗

Collection and Cryopreservation of Autologous Peripheral Blood Stem Cells and its Application in Autologous Peripheral Blood Stem Cells

摘要


背景及目的:自體周邊血液幹細胞移植前需要經過幹細胞收集、冷凍保存的步驟,研究自體周邊血液幹細胞之收集與冷凍保存技術及於本院施行自體幹細胞移植的成效與經驗。 方法:自2000年11月至2002年4月,共有12位病人利用CS-3000連續性血球分離機分離收集自體周邊血液幹細胞,將收集後的周邊血液幹細胞加入一些抗凍劑使成濃度為5.4% DMSO, 1.7% HESPAN, 3.33%人類白蛋白後,直接放置於零下80℃冰箱冰凍。病人經高劑量化學治療後,接受自體周邊血液幹細胞移植,利用病人周邊血液中性顆粒球絕對數目(absolute neutrophil count,簡稱ANC)及血小板數目恢復情況,評估幹細胞冷凍技術與自體周邊血液幹細胞移植的成效。 結果:共12位病人進行45人次周邊血液幹細胞分離術,得到52袋血品,其中23袋(44.2%) CD34(上標 +)細胞數小於0.5×10^6細胞/公斤病人體重,23袋(44.2%) CD34(上標 +)細胞數大於2.0×10^6細胞/公斤病人體重,6 袋(11.6%) CD34(上標 +)細胞數介於0.5~2.0×10^6細胞/公斤病人體重之間;有6位病人接受過一至四次的高劑量化學治療,回輸了10袋零下80℃保存的周邊血液幹細胞,周邊血液幹細胞冷凍保存的天數平均為108.7±103.4天(18~345天) , 每袋回輸於病人體內的幹細胞均能使病人ANC數於回輸後9~10天(9.7±0.4天)大於500細胞/微毫升,血小板數大於20,000細胞/微毫升於回輸後10~25天(13.7±4.2天)內恢復。 結論:對於一些罹患血液疾病或惡性腫瘤的病人,需經過高劑量化學治療才能將腫瘤細胞殺死,利用自體周邊血液幹細胞收集及不需程式降溫儀的幹細胞冷凍保存技術,應用於病人接受高劑量化學治療後的骨髓恢復,是一種安全可使用的治療方法。

Background and purpose: Autologous peripheral blood stem cell transplantation (APBSCT) requires the acquisition and cryopreservation of peripheral blood stem cells. We present our experience in the collection and cryopreservation of autologous peripheral blood stem cells, and describe the efficacy of APBSCT in patients at Changhua Christian Hospital. Methods: From November 2000 to April 2002, autologous peripheral blood stem cells were collected by apheresis in 12 patients using a continuous-flow blood separator. Cells were transferred to a cryopreserved bag with a solutions containing a final concentration of 5.4% DMSO, 1.7% HESPAN, and 3.33% human albumin and stored at -80℃ until further use. Six patients received APBSCT after high-dose chemotherapy. Absolute neutrophil count (ANC) and platelet counts were used to assess the outcomes of stem cell cryopreservation and APBSCT. Results: Fifty-two blood bags were harvested from peripheral blood stem cells apheresis in 12 patients. The CD34(superscript +) cell count was less than 0.5×10^6 cells/kg in 23 bags (44.2%), more than 2×10^6 cells/kg in 23 bags (44.2%), and between 0.5×10^6 and 2×10^6 cells/kg in 6 bags (11.6%). Six patients were treated with high-dose chemotherapy 1 to 4 times, and 10 cryopreservation bags of PBSC were thawed and infused to patients. Cells were stored for an average of 108.6±103.4 (range, 18 to 345) days. Every bag had a recovered ANC count of more than 500 cells/μL after 9 to 10 days (mean, 9.7±0.4 days) and a platelet count of more than 20,000 cells/μL after 10 to 25 days (mean, 132±4.2 days). Conclusion: Collecting and subsequent uncontrolled-rate freezing of peripheral blood stem cells offers a safe and reliable approach in APBSCT treatment of patients with blood disorders and malignant tumors after high-dose chemotherapy.
