The clinical picture of elevated intraocular pressure-induced interlamellar stromal keratitis (PISK) after laser in situ keratomileusis (LASIK) is identical to diffuse lamellar keratitis (DLK) after LASIK. However, PISK differs from DLK in being late onset, correlated with intraocular pressure (IOP) elevation and resolved with IOP lessening, and being worsened by frequent topical steroid therapy. We present a case of PISK after LASIK, which was initially treated as DLK. TOP elevation was noted one month after LASIK. After discontinuation of steroid therapy and lowering of IOP, symptoms subsided. It is recommended that IOP be measured in cases of LASIK in routine follow-up to rule out the possibility of PISK.
