Reconstruction of Digital Soft Tissue Defect Using Mini Dorsalis Pedis plus First Web Space Free Flap
應用迷你足背動脈自由皮瓣重建手部皮膚軟組織缺損
摘要
因為各種不同的傷害所導致的手部及手指皮膚及軟組織缺損常造成手部肌肉、骨骼、或肌腱的外露，而需使用不同的手術方式來加以覆蓋。傳統上這類的皮膚軟組織缺損可用皮膚移植、腹部皮瓣或帶血管莖的鼠蹊部皮瓣可以對手部組織缺損部位提供良好的皮膚軟組織覆蓋，但是因為較不能提供良好的感覺功能，故對於外來傷害的預防較不敏感。在外觀上有時也因為這些皮瓣的皮下組織肥厚而使重建手術後手部外觀較不被患者接受，也影響到手指活動的功能。使用皮膚移植手術雖有比較好的感覺功能，但是手術後移植皮膚的攣縮問題卻可能使得手部關節活動受到限制。 我們應用迷你型的足背動脈自由皮瓣對手指及掌部皮膚軟組織缺損作重建手術，術後手部關節活動情況良好，有較佳的外觀，感覺功能回復狀況也有較滿意的結果。

Small to medium skin and soft tissue defects of the hand and fingers due to injury have been treated with skin grafts, random abdominal flaps or pedicled groin flaps. These flaps offer good cutaneous coverage but do not provide cutaneous sensation to protect the hand from traumatizing external agents. Skin grafts provide better sensation but skin contraction may restrict the finger’s functional range of motion. Mini dorsalis pedis plus first web space free flap provide better restoration of sensation and appearance to the injured finger and is an excellent choice for reconstruction of skin and soft tissue defect of the hand and finger.
