Flow-Throung Medialis Pedis Flap for One Stage Reconstruction of Soft Tissue Defect and Revascularization of an Ischemic Finger-One Case Report

利用流穿(F1ow-Through)足內皮瓣同時來重建手指的缺血併軟組織缺損-病例報告

摘要


手指的嚴重創傷可使軟組織受損,並導致底層重要結構的暴露。若此時合併有指動脈的斷節性壓碎所造成手指缺血,則問題更形複雜。在此我們提出一個案例,利用足內皮瓣的流穿(flow-through)同時重建血流及軟組織之缺損。術後追蹤,病人第五指的外形、感覺及運動,均甚滿意。由這個例子,我們可以學到此皮瓣的幾個優點: 1.厚度薄,與手指有類似的顏色和構造; 2.俱有與手指相同的平滑性質的皮瓣(glabrous skin),使其有較好的感覺恢復; 3.提供流穿(flow-through)的通道; 4.供瓣區不明顯以及很少的併發症(morbidity)。

Severe trauma to the finger may cause extensive soft tissue damage and result in exposure of vital structures. The problem becomes even more profound when the finger is devascularized resulted from segmental crush of the digital arteries. We present such a case which we used a flow- through medialis pedis flap for revascularization as well as for reconstruction of coverage defect in one operation. From this example we learned a lot of advantages of this flap for one stage restoration of distal circulation and coverage defect reconstruction in the finger.
