Adult Descending Colocolonic Intussusception Caused by a Large Colonic Lipoma-Case Report
成人降結腸大脂肪瘤合併大腸腸套疊-病例報告
摘要
一例罕見由大腸脂肪瘤引起成人大腸腸套疊病例在此報告，術前即由大腸鏡檢查及腹部電腦斷層檢查臆診。此病例有左上腹疼痛及血便病史。下消化道鋇劑攝影檢查呈現降結腸充盈缺損及彈簧線圈徵相。腹部電腦斷層檢查可見降結腸內有脂肪密度影像，最後病人接受腸道部分切除手術及端對端吻合術。病人術後病情穩定恢復，並於術後15天出院。組織學檢查顯示該大腸腫瘤為降結腸黏膜下脂肪瘤。

A rare case of descending colocolonic intussusception originating from a large colonic lipoma was presented. A preoperative diagnosis was made by performing a colonoscopic examination and abdominal CT scan. The patient presented with symptoms of LUQ pain and bloody stool passage. Barium enema revealed a large filling defect in the distal descending colon with a “ coiled spring ”appearance indicative of intussusception and an abdominal CT scan demonstrated a fat density within the descending colon. We performed segmental resection with end to end anastomosis. The patient recovered without complications and was discharged 15 days following the operation. A postoperative histology confirmed the presence of a benign submucosal lipoma of the descending colon.
