Use of Transthoracic Three-Dimensional Color Doppler Echocardiography to Diagnose Congenital Aortic Arch Anomalies

使用三度立體彩色杜卜勒心臓超音波來診斷先天性主動脈弓畸形

摘要


背景:對先天性主動脈血管畸形的解剖構造之精凖判斷,是決定外科手術的主要根據。經胸前三度立體彩色杜卜勒心臓超音波可心提供主動脈結構民常特徵更大範圍及有效的評估。 目的:本研究旨在探 討應用多切面三度立體彩色杜卜勒心臓超音波所擷取的影像,來評估先天性主動脈弓畸形的解剖構造,以評仨其作為診斷工具的可行性與精確性。 方法:全部八位先天性主動脈弓畸形的病人(5.1±1.1歲)皆接受經胸前三度立體彩色杜卜勒心臓走避音波的影像擷取。藉由擷取連續多切面旋轉的二度心臓超音波影像,經重組後獲得一個完整的三度立體彩色杜卜勒心臓超音波影像,其容積範圍涵蓋整個主動脈弓其分支的血管。 結果:八位病人皆能完成三度立體彩色杜卜勒心臓超音波影像的擷取與分析。在完成三度立體彩色杜卜勒心臓超音波獨立診斷後,基結果再與其他診斷方法的結果相比較,以便確診,基他診斷方法包括核磁共振(MRI)、電腦斷層(CT)、心導管檢查或開刀。結果顯所有病人在使用三度立體彩色杜卜勒心臓超音波後其診斷結果與其他方法具有一致性。 結論:依據研究結果,針對先天性主動脈弓畸形的病人,使用三度立體彩色杜卜勒心臓超音波可以提供精確有診斷。故本研究建議除了核磁共振、電腦斷層或心導管檢查之外,三度立體彩色杜卜勒心臓超音波可以成為另一項有效的診斷工具。

Background: Surgical correction of congenital aortic arch anomalies (CAAAs) requires precise anatomic delineation. Transthoracic three-dimensional color Doppler echocardiography (TT3DCDE) might be a viable means of assessing the morphologic features of structural arch diseases. Objectives: This study evaluates the feasibility and accuracy of using TT3DCDE to clarify the anatomy of CAAAs. Methods: A total of 8 patients (5.1±1.1years) with CAAAs underwent TT3DCDE studies. Transthoracic rotational acquisition and reconstruction of sequential 2DE was used to obtain a TT3DCDE data set encompassing the entire aortic arch and branching vessels for volume-rendered TT3DCDE data set encompassing the entire aortic arch and branching vessels for volume-rendered TT3DCDE images. Results: Volume-rendered TT3DCDE images were successfully obtained and analyzed for all patients. Diagnosis based on TT3DCDE was made independently and compared with corroborative studies, including magnetic resonance imaging (MRI), computed tomography (CT), angiography or surgery. There was no discrepancy between TT3DCDE diagnosis and other imaging modalities and/or surgery. Conclusions: Our results suggest that TT3DCDE can be used for accurate diagnosis of CAAAs and may provide an effective alternative to MRI, CT or angiography for this group of patients
