照顧一位子宮頸癌行根除性子宮切除個案之護理經驗
Care of a Patient with Cervical Cancer Receiving a Radical Hysterectomy
摘要
近來國內女性因子宮頸癌需接受子宮切除手術日益增多，因此在照護方面逐漸地被重視。本篇報告為照護一位因子宮頸癌需接受根除性子宮切除個案之護理經驗。筆者於個案住院期間，運用Gordon十一項功能性健康型態進行評估，採用直接護理、觀察、會談、傾聽及電話訪談方式，收集相關資料，發現個案主要的護理問題有焦慮、身體心像紊亂、性功能障礙、尿滯留。於護理期間，筆者針對其問題擬定整體性、個別性及適切性的護理措施，同時給予關懷、鼓勵、讚美、陪伴與支持，幫助個案勇敢面對疾病及接受治療，重整身體心像，維持美滿的性生活。此外，於個案暫時無法正常排尿期間，筆者教導膀胱訓練方法，且出院後以電話訪談追蹤居家排尿，協助個案早日恢復健康。

Recently, an increasing number of women are receiving hysterectomies for the treatment of cervical cancer in Taiwan. This case report describes the nursing experience involved in treating a woman with cervical cancer requiring a radical hysterectomy. During her hospitalization, the Gordon's 11-events functional health pattern assessment guide was used to assess health needs. Direct care involved observing the patient's condition, interviewing, empathetic listening, telephone interviewing to collect the correlative data, identifying the main nursing problems, which included anxiety, body image disturbance, urine demurrage, and sexual dysfunction. At the same time, the patient was provided concern, encouragement, praise, companionship, and support to help her face her disease, accept treatment, rally a positive body image, and maintain felicitous sex life. Because the case was unable to urinate normally, she was taught bladder control. Telephone interviewing was used to follow up urination at home after hospital discharge and help the case regain normal life function.
