Aims: This paper aims at developing a comprehensive and effective home care system by providing home care skills support. A home care skill guideline focusing on wound care, nasal tube care and Foley tube care is proposed, while a case management guideline is also developed. Method: This is an action research. Home care nurses working at Worker's Medical Clinic of Macao Federation of Trade Unions were recruited to participate in this study. Field practice observation, survey and interview were conducted as initial assessments. Immediate feedback and training workshop were then provided after the assessment. The result was evaluated by a post-test observation and survey. Result: Home care professional knowledge and home care skills were improved after intervention. It was found that the participants were familiar with the definition of home care and roles of community nurses. However, their nursing competence and knowledge have to be improved in the future. There is an immediate need for related training and support. Conclusion: The home care nurses' knowledge and skills were improved in this study.
