A School-Based Health Promotion Program for Stressed Nursing Students in Taiwan

探討以學校為基礎的健康促進計畫成效-針對台灣承受壓力的護理學生

摘要


背景 護理學生面臨臨床及大學課業壓力,許多的理論及研究文獻建議,同儕的支持及規律運動是非常重要的,且可以有效的管理護理學生的壓力。目的 本研究旨在探討以學校為基礎的健康促進計畫,針對台灣護理學生,以團體運動方式介入及同儕的支持來管理壓力。方法 採類實驗研究設計,資料收集工具包括壓力問卷、半結構式問卷及團體討論,參與者為來自北部某技術學院的護理學生共77位,採隨機分配到實驗組(n=37)及對照組(n=40),計畫為期16週。參與者是中度及重度壓力的學生,實驗組分成小組參與團體運動,每週三次,每次30分鐘。在第八週後,研究者邀請實驗組各小組分享他們的感受及管理壓力的策略,實驗組及對照組皆需完成壓力問卷的前後測試。量性資料以SPSS 14版套裝軟體進行分析;團體討論的質性資料以內容分析法分析。結果 研究結果發現,實驗組護生的壓力顯著下降,且實驗組及對照組的下降程度有顯著不同。參與介入的護生比未參與者的壓力較低,並且對於同儕的支持及運動持正向看法。結論/實務應用 結果證實,以學校為基礎的健康促進計畫,對於減輕台灣護理學生的壓力是有效的,此研究可提供教師於發展協助護生管理壓力健康促進計畫的參考,更可協助學生藉由運動及同儕支持進而發展抒解壓力策略。

Background: Nursing students face both clinical and academic stress. Extensive theoretical and research literature suggests that peer support and regular exercise are critically important and can efficiently manage stress for nursing students.Purpose: The purpose of this study was to investigate the effect of a school-based health promotion program in a group physical activity intervention and peer support program for stressed nursing students.Methods: This study used a quasi-experimental design to collect information and collected data from a stress questionnaire, semistructured questionnaire, and group discussion. Participants included 77 nursing students at an institute of technology in northern Taiwan. Participants were randomly assigned into experimental (n=37) and control (n=40) groups. Program duration was 16 weeks. Participants were selected based on their assessment results as having moderate or severe levels of stress. All participants in the experimental group took part in a group physical activity for 30 minutes three times a week. Eight weeks later, the researcher invited each group to discuss their feelings and stress coping strategies. Both groups completed pretest and posttest stress questionnaires. Quantitative data were analyzed using SPSS 14.0 Statistical Package for Windows, and qualitative data from each group discussion were analyzed using content analysis.Results: Results revealed that level of stress was statistically decreased in the experimental group. Posttest stress levels were significantly different in experimental and control groups. The results suggested that students who participated in the intervention had less stress than did those in the control group after the intervention. Those in the experimental group held positive views of peer support and physical activity.Conclusions/Implications for Practice: The results of this study confirmed the efficacy of school-based health promotion programs in reducing stress in nursing students. Findings may provide educators with information to assist their developing effective health promotion programs to manage stress for their students. This study can also help students develop personal coping strategies through physical activity and peer support.
