病例報告:犬紅斑性狼瘡以hnRNP Gi重組蛋白協助免疫診斷

Case Report: Canine Systemic Lupus Erythematosus Diagnosed by the Usage of Recombinant HnRNP Gi Protein

摘要


一隻10歲齡、已中性化之雄性混種犬,因左前肢疼痛,腹部及鼠蹊部出現紅斑,及尿液顏色呈深棕色而被帶至中興大學獸醫教學醫院進行診治。尿液檢查發現潛血、白血球與高濃度蛋白,且尿渣含白血球、紅血球、鱗狀上皮細胞圓柱、移行上皮細胞圓柱、顆粒圓柱及細菌等。血液檢查顯示患犬血小板減少(thrombocytopenia) (33,000/立方公厘)。以間接免疫螢光染色法(indirect immunofluorescent assay, IFA)檢測,血清為抗核抗體(antinuclear antibody, ANA)陽性;而使用我們先前開發含G型核內異質核糖核蛋白(heterogeneous nuclearribonucleoprotein G, hnRNPG)主要免疫顯性區(major immunodominant region)之重組蛋白(hnRNP Gi)為抗原,以西方墨漬法分析時,我們發現血清中具有抗hnRNP G抗體。依據臨床病徵及血清分析結果,患犬呈現皮膚紅斑、腎臟病變(蛋白尿與細胞尿圓柱)、血液病變(血小板減少)、免疫病變(抗hnRNP G)及抗核抗體等症狀,確診為犬紅斑性狼瘡。

A ten-year-old castrated male mongrel was admitted to the Veterinary Teaching Hospital of National Chung Hsing University. The dog had sores in the left front leg, and erythema on the abdominal and groin area. The urine was dark brown, in which high protein levels, occult blood, and leukocytes were detected. In urine sediments, besides leukocytes and red blood cells, casts composed of squamous epithelium, transitional epithelium and granular cells, as well as bacteria were also observed. Blood examination revealed thrombocytopenia (33,000/mm^3). By employing indirect immunofluorescent assay, antinuclear antibody (ANA) was detected in the serum. Using a previously developed recombinant protein (hnRNP Gi), which contains the immunodominant region of heterogeneous nuclear ribonucleoprotein G (hnRNP G), as antigen for Western blot analysis, we further detected the presence of anti-hnRNP G antibodies in the serum. Based on these clinical manifestations and serological findings, including erythema, renal disorder (proteinuria and cellular casts), hematological disorder (thrombocytopenia), immunological disorder (anti-hnRNP G antibodies), and antinuclear antibodies, the dog was diagnosed as having canine systemic lupus erythematosus (SLE).
