我國疾病分類人員角色認知調查研究

A Survey of Role Recognition of Disease Coders in Taiwan

摘要


目標:本研究主要在瞭解我國疾病分類技術人員與健康保險相關人員之工作現況,期從工作實務現況依實證基礎進一步評估專業需求,並整理出較合適的工作模式,研究結果可提供疾病分類技術人員與健康保險相關人員之依循及相關機構參考,並於在職訓練課程、改善業務規劃等,提高醫院工作效率,使疾病分類業務的推行更加順利。 方法:本研究採郵寄問卷方式進行資料蒐集,依據台灣醫院協會與台灣病歷資訊管理學會,歷年疾病分類人員甄審考試公告及格人員名單爲研究對象,發出894份問卷,回收190份,回收率21.3%。 結果:疾病分類工作者以女性較多,佔97.9%,教育程度以大學畢業居多,佔46.8%,且以醫務管理居科系居多,佔52.1%,在醫療院所工作年資平均爲12.4年(SD=6.8),年資大於10年以上醫療院所工作年資,佔總數60.5%。考過疾病分類甄試者中,從事疾病分類工作平均年資爲7.1年(SD=4.5),年資大於10年以上疾病分類工作年資者,佔總數59.9%;另從事健保申報的人員中,平均年資爲5.9年(SD=3.9)。平均年所得爲483,716元,其中以服務於醫學中心之受測對象平均年所得居高,爲538,559元。疾病分類人員認爲每日最佳編爲數量平均爲每天27.9本病歷,而平均每本病歷編碼時間爲12.4分鐘,疾病分類人員認爲每本病歷理想的編碼時間平均爲18分鐘。普遍認爲單位疾病分類人員不足的佔52.2%。受測者中,對於目前從事職務工作負荷滿意度,40%感到不滿意或非常不滿意;對於目前從事職務整體工作滿意度,有42.6%感到滿意或非常滿意。

Objectives: This study was to find out the status quo work conditions of Taiwan's disease-coding technicians and health insurance-related personnel. We hoped to use their actual work situations as a factual foundation to further explore their expertise requirements, and put together more suitable work patterns for the subjects to follow. The findings can also be used as references for their employers to beef up their on-job training courses, upgrade their operational effectiveness, enhance hospital working efficiency, and make disease coding process much smoother. Methods: In this study, we rely on responses to a specifically designed questionnaire mailed to those who passed a special examination for disease coders in the past to collect needed data. Candidate name lists were gathered through the help of Taiwan Hospital Association and Taiwan Medical Record Management Association. A total of 894 questionnaire letters were sent out and 190 completed ones did come back and the responding rate is 21.3%. Results: Most of the responding disease coders are female (97.9%), college graduates (46.8%), and have majored in medical management (52.1%). Their average working time with a hospital is 12.4 years (SD=6.8). The percentage of all those disease coders who have worked at a hospital for more than 10 years is 60.5%, For those who have passed the disease coder examination, their average working time as disease coders is 7.1 years (SD=4.5), or 59.9% of them have worked as a disease coders more than 10 years. On the contrary, for those who work as National Health Insurance claimers, the average working time is 5.9 years (SD=3.9), and their average annual income is NT$483,716. Among them, people work at medical centers receiving a higher average annual income of NT$538,559 The respondents in this study claim that the optimized number for daily coding is abut 27.9 medical records and ideal time needed for coding each medical record is 18 minutes; however, in practice, the average time of coding is only 12.4 minutes. About 52.2% of the respondents claim that fewer than enough disease coders are there in their units. Regarding the satisfactory towards their workload, 40% of the respondents are unsatisfied or extremely unsatisfied. For the satisfactory towards their current overall job, 42.6% of the respondents feel satisfied or extremely satisfied.
