Do the Levels of Health Literacy Matter in the Health Promotion Intention? Report an Oral Cancer Screening Project in Taitung, Taiwan

健康知能與健康促進意願有關嗎?台東地區口腔癌篩檢計畫報告

摘要


口腔癌導致高死亡率,治療過程亦十分痛苦。透過口腔癌篩檢以早期發現是降低死亡率的有效方法。我國多年來即已提供免費的篩檢服務,但口腔癌仍高居癌症死亡的第六位,其主要原因即在於篩檢率仍然偏低。本研究探討個案之健康知能水準與其參與健康促進活動之口腔癌篩檢意願的關聯性。樣本取自口腔癌死亡率偏高的台東地區30歲以上居民。共有308份有效樣本納入分析。本研究之結果顯示,健康知能與個人意願隨部分人口變項而異。檢測結果顯示,健康知能程度與餐與口腔癌篩檢的意願高低有中度關聯。本研究認為除了政府單位所建構之易及性之外,政府單位應採取更進一步的措施,以提升國人之健康知能,進而提升國人對口腔癌篩檢的態度,方能對口腔癌篩檢有較大效果。

Oral cancer leaded to high mortality rate and a highly painful treatment procedure. Oral cancer screening (OCS) was an effective way in reducing the mortality through earlier identification. Taiwan offered such free service to the citizen for years, deaths caused by this cancer remained the sixth on the list. Main reason behind this was the low participation rate of OCS. This research explored the association of the peoples' behavioral intention and their levels of health literacy. Samples were residents who are 30 years old and above and lived in Taitung County in which the mortality rate of oral cancers is the highest in Taiwan. There were 308 valid questionnaires gathered for statistical analyses. This research revealed that the levels of health literacy and the strengths of intention were varied in some demographic variables. The test results also indicated that the health literacy and the intentions of taking OCS were moderately associated. Beyond the friendly accessibility the government had created, advanced procedures should be taken to improve the national's health literacy and to change the citizen's attitude toward the oral cancer screening. By dong this can help increasing the participation rate.
