照護一位多處骨折病患之復健護理經驗

Rehabilitation Nursing Experience of a Patient with Multiple Fracture

摘要


突如其來之車禍意外事件,經常造成骨折之結果,而肢體多處骨折常導致病患肢體乏力及長時間臥床不動。本文在探討一位多處骨折病患歷經創傷手術壓力後之因應調適過程,及面臨長時間的復健過程,如何協助病患調適執行完整復健計劃,促進回復肌力下床活動,儘早回到家庭工作崗位。筆者自2005年11月23日至12月11日,運用Gordon 11項健康功能型態評估病患之護理問題為急性疼痛、組織完整性受損、身體活動功能障礙、知識缺失、個人因應能力失調。筆者藉由衛教、會談及復健運動訓練,引導病患正確且完整執行復建計劃增進身體舒適度,漸漸恢復身體活動功能,進而協助早日回歸正常生活能力,以恢復獨立之生活。

Traffic accidents may cause multiple fractures, which might cause the patient's limbs to become weak and they may become bedridden for a long time. This study investigates how one multiple fracture is treated through wound operation in the correct way where the patient faces a long-term rehabilitation process, how to help patients adjust to temporary life changes and have courage in the rehabilitation plan, to develop the muscular strength to leave the bed, and finally get back to a normal family and work life position at the earliest opportunity. From November 23, 2005 to December 11, this study uses Gordon's 11 items of health function pattern attitude to assess the patient's nursing questionnaire in order to determine: 1) acute pain; 2) impaired tissue integrity; 3) physical function impairment; 4) knowledge deficit; and 5) an imbalance in individual response ability. Through a vigorous teaching, interviewing and exercise rehabilitation plan, by leading the patient to be rightly focused, results in gradual recovery of healthy activity functioning, therby helping the patient to resume normal viability as soon as possible, in order to resume an independent life.
