照顧一位不孕症外籍新娘執行腹腔鏡手術的護理經驗

Caring for an Infertile Foreign brid-a Nursing Experience in Executing a Laparoscopic Operation

摘要

本文是描述一位來自廈門的外籍新娘嫁入台灣五年來從未懷孕，這五年當中不斷求助於醫師，除了多次的誘導排卵外、亦執行了四次人工受孕、一次試管嬰兒；也接受中醫偏方的治療但仍不孕。此次因個案接受腹腔鏡手術於血液檢驗發現CA-125偏高(36u/ml)，醫師懷疑個案罹患子宮內膜異位症而建議入院治療。筆者於照護期間（94年3月7日至94年3月10日）藉由仔細觀察、良好的溝通及會談技巧收集資料，並應用羅氏適應模式來評估個案，進而資料分析和歸納，以確立個案的健康問題有：(1)無法受孕的挫敗感、(2)對醫療過程失去信心、(3)腹腔鏡手術所造成的疼痛。筆者希望藉此文獻探討及護理經驗的分享，使護理同仁日後照顧此類病患時，除給予手術前、後身體症狀的護理外，亦能重視其負向情感之問題，以達完整性之照護。

This article describes a foreign bride from Shia-Man who has been married and living in Taiwan for five years, but has yet to become pregnant. During this period, she has continually sought doctors' help, tried AIH for four times and IVF-ET once, other than multiple attempts at ovulation induction. She also received Chinese herbal treatment but has remained infertile. Recently, her blood test revealed an elevation in CA-125 (36 u/mL); endometriosis was impressed and she was recommended to be admitted for laparoscopic operation. During her stay in the hospital (May 7th 2005~May 10(superscript th) 2005), we collected data by observation and communication, then used the Roy Adaptation Model of Nursing to analyze and assess the situation of the patient; it was ascertained that the patient had the following problems: (1) frustration from being infertile; (2) loss of confidence in medical treatment(s); and (3) pain resulting from the laparoscopic operation. By discussing this article and sharing this experience, we wish to urge other colleagues to place more value on evaluation of psychological problems in addition to typical nursing care before and after operations when treating similar patients in the future.
