照顧一位口腔癌病患術後之護理經驗

摘要

本個案報告是探討一位48歲初次診斷為口腔癌的中年男性病患，術後面對生理上的不適、身體心像以及角色功能改變衝擊之護理經驗。照護期間為95年5月18日至7月4日，筆者於照護過程中運用Gorden十一項功能性健康型態為評估工具，確立個案有疼痛、營養狀況改變－少於身體所需、身體心像紊亂及角色扮演改變等四項健康問題。護理過程先針對個案生理問題，給予個別性的措施，改善營養及疼痛的生理問題，並依循處理心理層面問題。護理措施包括鼓勵個案表達心中感受、主動參與治療與處置、提昇自我照顧的能力，並教導家屬接納個案的情緒反應，使個案的生、心、社會問題都能得到適當的處理。期望藉此護理經驗之分享提供護理同仁照顧口腔癌病患之參考。

The purpose of this case study was to explore the nursing experience of a 48-year-old male patient with oral cancer. The patient was middle-aged and was diagnosed with oral cancer. The caring period was from May 18 to July 4 (2006). Gorden's functional evaluation of health conditions was used as a guide for data collection. Results identified four health problems-pains, imbalanced nutrition, disturbed body image and an ineffective role performance. Nursing intervention focused on management of physical problems by providing individual care in order to improve nutritional status and reduce pain. Nursing intervention included encouraging the patient's expression of feeling, actively involving treatment and care, enhancing the ability of self-care, and helping the family accept the patient's emotional reaction(s). The nursing experience from this case study can help nurses who are caring for oral patients in similar cases in the future, perform more effectively.
