協助一位初罹骨肉瘤病患接受化學治療之護理經驗

摘要

本文是敘述一位34歲男性骨肉瘤患者，在最初得知罹患癌症時所產生的心理衝擊及在接受化學治療期間，因化學藥物影響而引起的生理、心理、社會及靈性等層面變化，所衍生的問題探討與其護理；運用會談、觀察、傾聽及Gordon十一項功能性健康型態評估個案，發現個案主要健康問題有：焦慮／與對疾病不可預測的治療效果有關，噁心／嘔吐／與化學治療藥物有關，睡眠型態紊亂／對治療及疾病不確定有關和身體心像紊亂／與化療導致身體外觀改變有關。在護理過程中，筆者以同理心及真誠的態度，與個案及其家屬建立良好的治療關係，以護理專業的知識，有效協助個案降低心理的疑慮，並適時地減輕生理上的不適，促以正向健康的態度來因應所面臨的壓力並增進其適應感，使個案能在護理人員及家屬的支持陪伴下，勇敢面對疾病接受治療，並克服治療時所產生的各種不適問題。

The purpose of this article was to describe nursing experience in a patient of 34 year-old male with initial suffering from osteosarcoma to the psychological impact and side effects of chemotherapy. We focused on discussion about the problems, derived from psychological, physical, social and spirit change due to chemotherapy and then further concentrate on the nursing care process. By conversation, observation, listening and Gordon function evaluation items, we found the patient with major healthy problems as below: anxiety, nausea/vomit, sleeping pattern disturbance and body image change. In the process of the nursing care, I established the relationship of trust and support with the patient and his family first. By means of nursing professional, I helped the patient to decrease the psychological doubts, ameliorate physical discomfort and prompt him to make well adjustments while facing the pressure. In the assistance of nursing care and family support, patient can faced the disease to take therapy bravely and overcome all kinds of discomforts associated with chemotherapy.
