一位因眼內炎導致失明個案之護理經驗

Nursing Experience for a Result of Endophthalmitis as Cause for Blindness Cases

摘要


本文描述因眼內炎致雙眼失明之個案,在面臨失能的困境,而產生無助及焦慮。於2013年6月27日至7月29日護理期間,藉由觀察、訪談、身體評估、查閱病歷等方式收集資料,運用Gordon十一項健康評估,發現個案有感覺及知覺改變:視覺、家庭運作過程改變、焦慮、自我照顧能力缺失:沐浴、如廁、進食之健康問題。筆者運用同理心、關懷及陪伴,鼓勵個案表達內心感受,適時提供生理照護及情緒支持,使個案採取正向態度面對自我,並運用出院準備服務協尋相關機構,幫助個案重建,最終回歸社會。期望藉由本文提供臨床護理人員主動發現及預期中途失明者面臨的衝擊,及時提供身、心、社會方面的支持與協助,重視中途失明者資源轉介回歸社會的議題。

This paper describes a patient with binocular blindness due to endophthalmitis who experienced helplessness and anxiety when facing physical disability. During the nursing period from June 27 to July 29 in 2013, data were collected through observation, interview, physical assessment, and chart review based on Gordon's 11 Functional Health Patterns. The health problems were identified as follows: disturbed sensory perception: visual, interrupted family processes, anxiety, and self-care deficit: bathing, toileting, feeding. During the nursing process, the authors employed empathy, attention, and companionship to encourage the patient to express inner feelings, provided timely physical care and emotional support to help the patient develop positive attitudes in self-confrontation, and applied discharge planning to help the patient find related institutions and receive rehabilitation, thereby facilitating their eventual return to the community. This paper could serve as a reference to help clinical nurses proactively detect and expect the impact of adventitious blindness, provide timely physiological, psychological, and social support and assistance, and emphasize the issue of resources referral and returning to the community in adventitiously blind individuals.
