摘要

單位護理人員照護腎臟移植病患過程中常聽病患及家屬詢問傷口照護、飲食指導及居家照護事項且問題重複詢問度高，也曾接到來電詢問照護事項，統計2011年8月至11月間共有8件來電或當面詢問相關照護問題，可見病患接受腎臟移植術後及居家護理指導後的理解程度，及護理指導的成效仍需進一步探討，因此激發此專案改善動機。其目的在經由了解腎臟移植術後護理指導現況加以分析檢討，期望改善護理指導完整性提升病患居家自我照顧能力。自2012年1月15日至8月31日藉由擬訂照護標準及護理指導手冊、重新規劃照護比例、整合資源醫療團隊合作、舉辦教育訓練等措施，結果腎臟移植術後護理指導完整性由36.67％提升至98.3%，達本專案目標值，確實提升護理指導完整性及病患居家自我照顧能力。

When we care for patients with renal transplant, the same issues will be raised to discuss with their family such as ＂wound care, dietary guidance and home care＂. Between August 2011 to November 2011, there were a total of 8 incidences of patients and their families asking questions related to those issues, whether they were at home or still in hospital. The purpose of this review is to analyze the current situation about nursing care of cases with renal transplant through the understanding of renal transplant care guide status. Material and method From 01 January 15 to August 31 we expected to improve patient self-care skills by completing our nursing care guide. With the preparation and nursing care standards guidebook, the proportion of re-planning of care, integration by the medical team of resources, and organization of education and training and other measures, the said improvement was successfully carried out. Result The results of renal transplantation care guide completeness increased from 36.67% to 98.3%, which reached the target and also upgraded care guide integrity. Conclusions Our proposed paradigm proved to be effective and well-rounded in improving nursing care and reaching above the upgraded care guide integrity.
