摘要

本文旨在運用Watson理論於一位Guillain-Barré Syndrome患者與其主要照顧者的護理經驗。加護病房護理期間為2012年3月16日至4月4日，個案因病情的急遽變化使其出現負向情緒反應，案夫亦因此承受巨大身心壓力，因而引發筆者探討的動機，經健康評估後發現個案有呼吸道清除功能失效、活動無耐力、便祕、焦慮等健康問題，而案夫亦有焦慮的健康問題，促使筆者運用Watson的十項關懷照護因素，以科學性問題解決方法改善個案呼吸道清除功能失效問題；協助並教導案夫共同為個案執行肢體復健，改善其活動無耐力問題；藉由腹部按摩運動、以同理心主動協助清潔，解決個案便祕問題；給予個案與案夫情緒支持、分享成功案例，透過夫妻間的情感交流，緩解個案與案夫的焦慮感。期許經由此護理經驗的分享，提供臨床護理人員日後在照護相似案例的參考。

This article describes a nursing care experience about a patient suffering from Guillain-Barré Syndrome and her primary caregiver through applying Watson’s theory. The care period was when the patient was in the Intensive Care Unit from March 16 to April 4, 2012. Due to the condition worsening, the patient elicited negative emotional reactions, and her husband was placed under great stress. The identified problems included ineffective airway clearance, activity intolerance, constipation and anxiety, while her husband also experienced anxiety. The ten primary caring factors of Watson’s theory were used to solve these problems. The patient’s respiratory function improved, while teaching and assisting the patient’s husband to help the patient with rehabilitation exercises increased the patient’s lower limb activity. Applying abdominal massage and assisted cleaning after bowel movement resolved the patient’s constipation problems. Giving emotional support, sharing successful case experience and assisting the couple in communicating successfully reduced the patient and her husband's anxiety. This nursing experience can serve as a reference for other clinical nurses facing similar situations.
