此文描述一位病人因出血性腦中風導致失能,除了身心承受煎熬外,對家庭運作也產生極大的衝擊。筆者於2015年10月12日至11月7日照護期間,依生理、心理、社會及靈性四層面進行整體性的評估,確立個案有身體活動功能障礙、無力感及家庭因應能力失調之健康問題,同時運用跨領域團隊合作照護模式,藉由各職類之專業人員共同協助下,及時協助病人及家庭成員能適應日常生活型態之改變,進而重返社會及家庭。希望藉此案例,提供照顧此類似病患時,能多加以利用跨領域團隊合作照護模式,整合各職類之專業,進而提供更完善之照護品質。
This case report describes how a patient with hemorrhagic stroke suffered physical and mental dimensions because of his physical disability. Besides, the disability also had impact on his whole family. During the period of caring from October 12, 2015 to November 7, 2015, the patient was holistically assessed for his physical, psychological, social and spiritual aspects. The problems that we identified included impaired physical mobility, powerlessness, and ineffective family coping. An inter-professional caring model was introduced. We assisted this family in adapting to the changes of lifestyle and returning to society through intervention by different medical professional staff members. The article provides an example of caring for similarly affected patients by inter-professional collaboration and improving the quality of care.
