照顧一位腰椎手術後併發非預期出血性小腦中風病人之加護經驗
The Nursing Experience in a Stroke Patient After Lumbar Surgery at Surgical Intensive Care Unit
摘要
本文係描述腰椎手術後併發非預期出血性小腦中風病人之加護經驗。護理期間自2016/7/8至2016/7/22，個案因腰椎手術後發生罕見的出血性小腦中風，引發筆者撰寫動機。筆者透過直接照護、身體評估、觀察、會談及病歷查閱等方法，對個案進行生理、心理、社會及靈性整體性評估，彙整後確立健康問題為：（一）腦組織灌流改變；（二）身體活動功能障礙；（三）家庭因應能力增進的準備度。藉由避免顱內壓上升的護理活動來維持正常的顱內壓，運用跨領域醫護團隊合作模式擬定個別性的護理措施，協助個案及家屬面對中風導致身體失能所造成身、心理的衝擊，協助個案增進自我控制感與自信心來面對疾病衝擊。藉此照護經驗的分享，提高臨床護理人員之警覺性。

The purpose of this report was to present the nursing experience and case of an unexpected cerebellar hemorrhagic stroke with acute hydrocephalus patient after lumbar spine surgery. The patient had been suffering from an emergent external ventricular drainage and stayed in the Surgical Intensive Care Unit from July 8, 2016 to July 22, 2016. During the case period, data were collected for attendance, evaluation, observation, interview, search of medical records, etc. An integral evaluation about the patient's physiological、psychological、social and spiritual states was also noted. The author noticed the following major nursing problems including ineffective cerebral perfusion、physical activity dysfunction、and family resource preparation for disability; then, the author provided nursing care to maintain normal intracranial pressure, and with a sound therapeutic relationship and applied interdisciplinary medical/nursing team, drafted an individualized and appropriate nursing plan for nursing personnel to face the psychosocial impact of the patient disabilities. Nursing staff also actively participated in the rehabilitation treatments to improve daily activity and confidence, as this was deemed necessary to empower a more positive attitude in the future.
