一位學齡前期兒童初次住院之護理經驗

A Nursing Experience of a Pre-school Child at First Hospitalization

摘要


本篇為一位學齡前期兒童初次住院之護理經驗。照護期間為106年6月22日至6月28日,以生理、心理、社會、靈性四大層面為評估架構,並藉觀察、身體評估、非結構式會談與傾聽收集資料,確立護理問題為:呼吸道清除功能失效與恐懼。護理過程,運用治療性遊戲,如:彩色圖卡、卡通動畫、貼紙、布偶擬人化與吹氣球遊戲等,與其建立護病關係,抒發、緩解恐懼,亦指導案母治療性遊戲運用與胸腔物理治療的執行技巧,讓案母與護理人員共同參與照護,促進個案配合完成治療處置,使殷殷期盼返家的個案恢復健康,順利出院,達全人照護之目標。為善用治療性遊戲於學齡前期兒童初次住院照護的成果,期盼分享此經驗,提供日後護理人員照護之參考。

This article describes a nursing experience of caring for a pre-school child during the first hospitalization. The nursing period was from June 22nd to June 28th, 2017. Data were collected from observation, physical examination, unstructured interview and listening to the patient with psychological, physiological, social and spiritual aspects as the assessment framework. The results of nursing diagnosis revealed ineffective airway clearance and fear. During the nursing process, therapeutic games like color cards, animated cartoons, stickers, anthropomorphic dolls, balloons blowing and so on were utilized to establish the nurse-patient relationship so that the patient could express and also ease their fear. In addition, the young patient's mother was also instructed to use the therapeutic games and skills for chest physical therapy so that the mother could care for the patient together with the caregiver to facilitate the patient's cooperation on the treatment, so that the patient could regain health and be discharged smoothly, reaching the goal of holistic health care. The author shares this experience of caring for a pre-school child in the first hospitalization for nursing staff to have a model reference in caring for such patients in the future.
