背景:在台灣65歲以上老人有活動式假牙比例為33.7%,假牙有粗糙或細微孔洞表面,因清潔不良的食物殘渣留存,而使細菌累積、刺激黏膜,導致念珠菌繁殖感染致病,但目前文獻對於假牙清潔方式、清潔液並無定論,且國內尚無整合性之文獻評價清潔成效。目的:以系統性文獻回顧方式探討以Chlorhexidine預防假牙中口腔念珠菌感染之成效。方法:搜尋UpToDate、Medline、The Cochrane Library、Embase、PubMed及華藝線上圖書館,於2019年8月至10月以「假牙」、「口腔清潔」、「Chlorhexidine」與「口腔念珠菌感染」等中、英文關鍵字,共篩選出3篇文獻,以Critical Appraisal Skill Program評估文獻品質。結果:將活動式假牙浸泡於0.12%到0.2%的Chlorhexidine清潔液中8小時、持續4天並合併刷洗,可有效減少假牙中的口腔念珠菌。結論/實務應用:建議可用含Chlorhexidine的清潔液,在社區與臨床上做推廣,也可進行大規模研究,以有效改善假牙族群的口腔衛生。
Background: In Taiwan, the proportion of movable dentures in the elderly over 65 years of age is 33.7%, with dentures frequently having rough or fine pore surfaces, caused by poorly cleaned food residues, thereby enabling bacteria to accumulate and stimulate mucous membranes, eventually leading to Candida reproduction and infection. There is no conclusion in the current literature about denture cleaning methods and solutions, and no systematic review has been conducted to explore the effective of cleaning. Objectives: To investigate the effectiveness of denture cleaning with Chlorhexidine in the prevention of oral Candida infection in a systematic review. Methods: UpToDate, Medline, The Cochrane Library, Embase, PubMed and Airiti Library documents were searched from August to October 2019, and by using the Chinese and English key words "denture" AND "oral cleaning" AND "chlorhexidine" AND "Oral Candida Infection", a total of 3 studies were detected. The Critical Appraisal Skill Program was used to assess the methodological quality. Results: Soaking dentures in a solution of chlorhexidine 0.12%-2% for 8 hours over four days together with brushing significantly reduced oral candidiasis in dentures. Conclusions/ Practical application: It is suggested that mouthwashing with chlorhexidine could be used for community and clinical promotion, and large-scale studies are required to be carried out to effectively improve the oral hygiene of the denture population.
