維持皮膚完整是護理品質重要的指標。本文旨在降低兒科加護病房護理人員使用醫療黏膠,引起病嬰(童)顏面損傷的發生率,並提升護理人員預防黏膠損傷的認知與正確的技術執行。經由現況分析發現,造成黏膠引起皮膚損傷高的原因,包括:未定期舉行醫療黏性相關皮膚損傷之課程、管路照護標準未依病嬰(童)特性選擇適合黏膠、無預防黏膠潮濕措施,使得更換頻率太高及不知道如何運用皮膚輔助產品。透過舉辦教育訓練、修訂照護標準、建立醫療黏膠使用照護指引及增設預防黏膠潮濕等改善措施,其醫療黏膠引起顏面皮膚損傷率由51.4%降為9.1%、護理人員對預防的認知率由40%提升至100%及執行醫療黏膠產品之正確率由43.2%提升至100%。本專案之成效可提供兒科加護病房臨床照護之參考。
Maintaining skin integrity is an important indicator of the quality of care. This project aimed to decrease the incidence of medical adhesive-related face skin injuries (MARSI) in a pediatric intensive care unit and to enhance the recognition of MARSI. After situation analysis, the main reasons for MARSI in our unit were as follows: (1) relevant education and training of staff was lacking; (2) standard operation procedures were not updated; (3) staff unfamiliarity with use of skin supplement products resulting in high frequency of changing adhesives; (4) staff being unaware of how to use such products. Through the following strategies and implemented programs including introducing protocols for applying medical adhesives, method of application guidelines, patient skin protection guidelines, as well as education for the prevention of skin damage through tape application and removal techniques, the incidence of MARSI obviously decreased from 51.4% to 9.1%; additionally, the recognition rate of nurses in preventing MARSI was significantly elevated from 40% to 100%, which perfectly achieved the target of this project. In conclusion, our present findings could act as a reference for clinical nurses in the pediatric intensive care unit.
