此文為運用治療性遊戲照護初次住院即入住加護病房的學齡前個案,護理期間為2019/11/18-11/22,運用Gordon十一項功能性健康型態評估進行資料收集後,確認其主要健康問題為:體溫過高、無效性呼吸道清除功能、恐懼,故針對學齡前兒童的發展特性、個案喜好的吹泡泡及火箭遊戲等,擬定個別性照護措施,並結合指導性、生理健康促進性與情緒宣洩性之治療性遊戲種類,協助面對住院的壓力調適、緩解對不熟悉環境及治療的恐懼,同時也邀請家屬參與此過程,一同訂定適合且具體的規範及獎勵,使個案增加安全感並配合治療,讓呼吸道清除功能儘早復原。期望分享此經驗增加爾後此類個案之加護護理經驗。
This article describes the nursing care experience of therapeutic play for a preschool child who was admitted to the intensive care ward for the first time in the pediatric ICU. The nursing period was from November 18^(th) to November 22^(th), 2019. With Gordon's 11 functional health patterns assessment tool and the patient's physical assessment, the patient's health problems were determined to be "ineffective airway clearance", "hyperthermia", and "fear". In response to these nursing problems, individual care measures were formulated based on the developmental characteristics of preschool children, the case's preference for blowing bubbles and rockets, etc., and combining instructional and physical health promotion and emotional catharsis therapeutic games to assist the patient in facing the pressure of hospitalization while alleviating the fear of unfamiliar environment and treatment, and at the same time by inviting family members to participate in this process to set appropriate and specific regulations and rewards together, so that the case could increase the sense of security and cooperate with the treatment, enabling airway clearance function to recover as early as possible. It is hoped that sharing this nursing experience could enhance the intensive nursing experience of this kind of care in the future.
