呼吸器是加護病房中使用頻率最多、成本最高的醫療儀器,呼吸器的耗損率,威脅著病人安全,反應著醫療成本。本專案運用問題確認導向做重點管理與提案改善,經特性要因分析後,擬定對策,針對加護病房呼吸器損壞率之劇增,改善其儀器管理與成本控制,及提升護理人員之專業素養。實施期間自2004年12月至2005年3月,其成效可見呼吸器耗損率由劇增的3.50%~11.32%改善至目標值1.5%以下;呼吸器修繕費用由7814元/月,降至280元/月;護理人員呼吸器的使用正確性,稽核結果由76.21%提升至99.6%。這些成效證明所採用的手法,可在不影響醫療品質的前提下,有效的降低加護病房呼吸器的耗損率與儀器成本,增進照護品質與病人安全,並提升了醫院人員成本概念,幫助醫院之營運。
Ventilators are the most frequently used medical instrument with the highest cost in an ICU. As the damage rate to ventilators increases, the risk to patients also increases and medical costs rise. This problem-based project was designed to emphasize resource management and to draft improved methods for avoiding damage to ventilators. Using cause and effect analysis, we devised plans to increase nurses' professional knowledge regarding instrument management and cost control. The trial period was from November 2004 to March 2005 and the results were as follows. First, the rate of ventilator damage decreased from 3.50%~11.32% to under the target set at 1.5%. Second, expenses for repairs decreased from 7,814 to 280 NT dollars per month. Third, the operational accuracy of the ventilators improved from 76.21% to 99.6%. Based on these results, this project has been demonstrated to decrease the rate of damage to ventilators and to lower costs associated with maintaining medical instruments. Moreover, by doing so we improved the quality of nursing care and patient safety while contributing to cost control for hospital professionals as well.
