提升急診室護理人員執行手術前護理準備完整性
Enhancement of Completion of Pre-operative Care in Emergency Department Nurses
摘要
急診室專科性多且屬性不同，完整的術前準備，對病患在手術過程中的安全密不可分。根據本院護理部品質管理委員會訂定之術前護理準備完整性之標準須達100%，經統計本單位2007年1月到2007年12月術前護理準備異常為27件，手術前護理完整性認知調查僅達91.87%，而護理人員執行術前護理完整性僅94.35%。本專案改善方法包括：安排單位人員在職教育、提供病患及家屬手術前準備衛教單張、建立資訊輔助系統運用（提醒未完成的項目及異常檢查結果之警示）、建立逐項核對制度。經由改善方案實施後，護理人員執行術前護理完整性提升至100%，術前護理異常件數由27件於2008年3月到2008年12月降為0件，本專案在臨床護理工作有顯著幫助，將可維護病患安全與增進急診照護品質。（志為護理，2012；11：1，85－96）

Due to diverse specialties and contrasting attributes in emergency room, complete preoperative care is inseparable from the intraoperative safety of patients. According to the regulation drafted by the quality management committee of the nursing department, the standard rate of complete pre-operative care must reach 100%. Based on the statistics, from January through December 2007, 27 cases of pre-operative preparations error were reported; the cognition on complete pre-operative care among ER nurses was only 91.87%; moreover, the implementation of complete pre-operative care by nurses was only 94.35%. The enhancements recommended were: planning on-the-job training programs, providing handouts to patients and their families regarding pre-operative care, establishing an informational support system (to remind unfinished items and to warn of abnormal test results) and an item-by-item checking system.After the implementation of enhancement plan, the completeness of pre-operative cares in nursing staffs was elevated to 100%; and the cases regarding pre-operative care error were reduced from 27 to 0. The project had significant positive results on clinical nursing practice in ER, and could be implemented to ensure patient safety and to improve the quality of emergency care.
