協助兩位照顧者提供中風病人連續性照護之居家護理經驗
A Home Health Nursing Experience of Assisting Two Caregivers in Providing Continuous Care for a Stroke Patient
摘要
本文描述一位二次中風個案由兩位兒子輪流照顧的護理經驗。因照顧者錯誤觀念且照顧方式缺乏一致及溝通，導致個案出現傷口癒合緩慢及潛在泌尿道感染的問題，照護期間自2009年7月14日至2010年1月31日，筆者運用Gordon十一項評估發現個案有照顧者輪替照顧連續性不足、營養狀況改變/體重過輕、皮膚完整性受損、潛在危險性感染等四項護理問題。利用電訪及家庭訪視的過程，與家屬建立良好的互動及信任感，澄清及統整兩位兒子的照護知識與技能，協助建立書面照護交班，同時提供管路、傷口照護衛教及回覆示教，另運用書面記錄協助案子與醫師溝通，並轉介營養師等護理措施，使個案傷口癒合、達到理想體重，改善個案的問題並提升案家的生活品質。（志為護理，2012；11：3，109－118）

This article described a home health nursing experience of assisting two caregivers in providing continuous care for their mother, who went through stroke twice and became bed-ridden after discharged from the hospital. Due to nursing misconceptions, inconsistent caring methods and lacking communication between the care givers, the patient exhibited signs of slow wound healing and potential urinary tract infection. From July 14, 2009 to January 13, 2010, the Gordon's eleven-functional-health patterns were applied, and through direct nursing care, observation, physical examination, interview, listening, interaction and communication with patient, four health related problems were indentified: (1) Inadequate continuous care by the two caregivers; (2) Altered nutrition/underweight; (3) High risk of urinary tract infection and (4) Impaired skin integrity. The home health nurse established a positive interaction and trusts with both caregivers through phone interviews and home visits, clarified and integrated the caregivers' nursing knowledge and techniques, established nursing shift report, and provided tubes, wound care education and returned demonstration. In addition, nursing measures such as written record was implemented to assist the caregivers in communicating with the physician, and a nutritionist was referred. Consequently, the patient's wound healed, body weight achieved ideal status and the patient's quality of life was improved. (Tzu Chi Nursing Journal, 2012; 11:3, 109-118)
