本文描述一位11個月先天膽道閉鎖女童接受活體肝臟移植手術及其父母的照護經驗。照護期間於2010年3月15日至20日進行護理評估,藉由會談、觀察、直接護理等技巧收集資料,確立家屬於手術前有照顧者焦慮、個案於手術中有潛在危險性體溫調節失常及潛在危險性損傷主要健康問題。術前提供家屬支持教育性措施,增進家屬對個案即將接受活體肝臟移植手術的認知及因應能力。術中提供完全代償性護理,預防手術的潛在危險性損傷。術後至加護病房,針對個案手術後疼痛問題進行討論。本篇個案報告運用手術全期照護的理念,協助個案及家屬渡過手術期間所面臨的健康問題,使個案能平安順利進到下一階段的照護,希冀本文從家庭整體性的照護過程,可提供作為日後照顧此類病童及家屬時之參考。
The article described a nursing experience of a family with an 11 month old girl with congenital biliary atresia undergoing living-donor liver transplantation. From March 15 to 20 in 2010, the author conducted a comprehensive nursing evaluation and collected data via interview, observation and direction nursing care, confirming that the main health issues were: family members experiencing caregiver anxiety prior to the surgery and the patient had potentially risk for thermoregulation disorder and injury during surgery. First, author provided the family with supportive educational measures, enhancing the family cognitive and the coping ability for oncoming living-donor liver transplantation. Second, wholly compensatory nursing and protective measures were applied to prevent potential risk for thermoregulation disorder and injury condition during surgery. Third, as the patient was transferred to the intensive care unit after the surgery, a discussion was held regarding postoperative pain control. In this article, the author employed the concept of perioperative nursing, aiding the patient and the family to overcome health issues faced during the surgery, allowing the patient and the family to proceed smoothly to the next phase of nursing. We hope this article regarding family-oriented nursing can be used as a reference when encountering similar patients and families in the future.
