描述中國籍十一個月大嬰兒,在父母陪伴下跨境行人工電子耳植入手術,因對陌生的醫療環境、且父母對於手術是否成功感到極端緊張、無助,引發收案動機,照護期間2017年9月4日~9月9日,以個案發展、生理、心理、社會及靈性整體性評估為主要架構,藉由直接照護、會談、觀察等方式蒐集資料,確立的健康問題有三:1.害怕、2.高危險性皮膚完整性受損及神經損傷,3.與照顧者角色緊張;綜合以上給予措施有:案母及心愛小物陪伴、提供安全防護、藉由引導以宗教祈求方式降低緊張及對父母之相關協助等。建議可以建置跨國際溝通平臺,藉由影音衛教、模型針具、聽診器及幫布偶戴手術帽等互動過程,使個案能獲得持續性、高品質的照護。
This article presents the perioperative nursing experience of an 11-month-old infant from mainland China whose parents brought her to Taiwan for cochlear implantation surgery. Parents felt extremely nervous and vulnerable due to the unfamiliar medical environment and risks inherent in the operation. The main assessment framework was based on physical, psychological, social, and spiritual integrity based on the Human Development Stage theory. Data were collected with direct nursing, individual interviews, and observation. The period studied lasted from September 4-9, 2017. Three main health problems were identified: fear, high-risk of impaired skin integrity and nerve damage, and Caregiver Role Strain. Authors provided the patient's mother with encouragement and kept familiar toys available for the patient to reduce tension and anxiety. Religious counseling and guidance was also provided for the parents during the perioperative period. Establishment of an international communication platform would ensure continuous and high-quality care for such cases. In addition, interactive procedures such as audio-visual education, model needles, stethoscopes, and simple interactive activities such as putting surgical hats on puppets can enhance the patient-nurse relationship, reduce the difficulty of clinical care, and improve the quality of perioperative nursing.
