一位燙傷併潛在危險性廢用症候群病人之護理經驗

A Nursing Experience of a Burn Patient at Risk for Disuse Syndrome

摘要

本文為探討一位燙傷併潛在危險性廢用症候群病人之護理經驗。病人因不慎被滾燙醬油燙傷導致雙下肢含十根腳趾2-3度佔全身體表面積30%燙傷，護理期間自2013年11月22日至2013年12月20日，筆者藉由收集病史、會談、病歷及身體評估等方式收集資料，以Gordon十一項健康功能型態進行整體性評估，確立病人有組織完整性受損、疼痛、潛在危險性廢用症候群、焦慮等健康問題。因無法了解病人出院後復原狀況，建議針對返家後追蹤可在出院計劃增列特殊交辦事宜，由門診護理人員持續追蹤成效，或藉由陽光基金會等社會團體，計畫病人長期整體性的復健歷程、居家生活及社會適應評估，以提升生活品質。

This article described a nursing experience of a major burn patient who was at risk for disuse syndrome. The nursing period was from November 22th to December 20th, 2013. Data was collected by means of observation, interview, medical records, and physical assessments. Using Gordon's 11 functional health patterns as framework for holistic assessment, the result revealed that the patient had impaired tissue integrity, acute pain, anxiety, and risk for disuse syndrome. As tracking patient recovery after discharge was impossible, we recommended the inclusion of post-hospital follow-up in the discharge planning that allows outpatient nurses to track the patient's recovery; or the planning of a long-term holistic rehabilitation, and home and social adaptation assessment in collaboration with NGOs like Sunshine Social Welfare Foundation to ensure the patient's life quality.
