提昇眼科手術病人出院護理指導完整率之改善專案
Improvement of Discharge Nursing Practice in Ophthalmology Surgery Patients
摘要
本專案為提昇眼科手術病人出院護理指導完整率，因護理人員執行出院護理指導內容缺乏一致及完整性，導致眼科手術病人出院護理指導完整率65.1%。現況發現：護理人員對出院護理指導時機不一致、指導內容不一致、點藥技術無回覆示教、衛教單張可近性不足、藥袋標示不完整。藉由修訂眼科出院護理指導標準作業流程、製作病人自我照護QR Code學習護照、張貼點眼藥海報及小卡、規劃藥籤QR Code。結果：出院護理指導完整率由65.1%提昇為98.6%；病人（家屬）出院自我照護認知由64.1分提昇為98.1分，病人對護理人員執行出院指導滿意度由71.4%提升至99.1%，達本專案目的。

This project upgrades the completion rate of the nursing discharge practice of nurses on discharged ophthalmology surgery patients. The lack of consistency and completeness rate in discharge nursing leads to a completeness rate of only 65.1% for discharged ophthalmic surgery patients. Upon investigation, it was found that the nursing staff inconsistently discharges nursing practice at each time point, knowledge on discharge nursing is inconsistent, incomplete knowledge in teaching technology, health education leaflets are not enough, and drug bag is incomplete. Consequently, this project revised the standard operating procedures of ophthalmic discharge care, manufactured patients' self-care QR code study passport, posted disinfection, and planned drugs with assigned QR codes. After the implementation of the project, the ophthalmology surgery patients' completion rate improved from 65.1% to 98.6%; patient (family member) self-care cognition improved from 64.1% to 98.1%; and hospital satisfaction increased from 71.4% to 99.1%. Thus, the purpose of this project was achieved.
