多媒體護理指導對住院中精神科病患提升戒菸認知、健康信念與自我效能成效之探討
The Effectiveness of Multimedia Nursing Guidance on Enhancing Smoking Cessation Knowledge, Health Beliefs, and Self-Efficacy Among Hospitalized Psychiatric Patients
摘要
醫院為禁止抽菸場所，精神科病房環境為封閉性環境，且精神科病患吸菸率為一般病患之兩倍以上。本文為多媒體護理指導對住院中精神病患的菸害認知、健康信念及戒菸自我效能之成效探討，以提升病患戒菸意願及菸害認知。本研究採開放性試驗設計，以立意取樣於北部某區域教學醫院精神科病房進行收案，隨機分派實驗組15人與控制組15人。以自填試問卷進行資料蒐集，內容包括戒菸個案專用紀錄表、尼古丁成癮度測量表、菸害知識、戒菸健康信念、戒菸自我效能量表。研究顯示，多媒體護理指導之實驗組比傳統護理指導對照組，顯著提升精神科病患戒菸認知及拒菸自我效能，而及健康信念則未顯著提升。本研究可作為各醫療院所護理人員進行精神科病人菸害防制護理指導之參考。

The psychiatric ward is a closed environment where the smoking rate among patients is more than twice that of general patients. This study aims to explore the effectiveness of multimedia nursing guidance on smoking-related knowledge, health beliefs, and self-efficacy for smoking cessation in hospitalized psychiatric patients, in order to increase their awareness of smoking hazards and enhance their willingness to quit smoking. The study adopted an open trial design and recruited 15 participants for the experimental group and 15 for the control group through purposive sampling from a psychiatric ward in a teaching hospital in northern Taiwan. The data included the Demographic Variable Statistics, Fagerstrom test for nicotine dependence, Tobacco Cognition Questionnaire, Health Belief scale, and the Self-Efficacy of Anti-Smoking S1cale. The study revealed that the experimental group, which received multimedia nursing guidance, demonstrated significant improvements in smoking cessation knowledge and self-efficacy compared to the control group that received traditional nursing guidance. However, there were no significant improvements observed in health beliefs for smoking cessation among the psychiatric patients. This study can serve as a reference for healthcare professionals in various medical institutions to provide smoke control and nursing guidance to psychiatric patients.
