A Case Report of Traditional Chinese Medicine Treatment in Idiopathic Sudden Sensorineural Hearing Loss

摘要

突發性耳聾為耳鼻喉科臨床常見的疾病，主要症狀為突發聽力受損，大多病患除了聽力減退之外，還有耳鳴、眩暈的症狀，因突發性耳聾的病理機轉仍然不明確，病毒感染，血管栓塞、耳蝸內細胞壓力反應以及免疫調節機制等理論眾多，臨床上治療方式選擇雖多，但療效仍不一定明確，許多病患仍持續為此病症所苦。本醫案為一46歲女性病患，從發病開始即在本院耳鼻喉科就診，純音聽力測驗診斷為突發性耳聾，經過西醫藥物治療一個月後仍未有明顯改善，因此前來中醫部求診，在接受針灸治療之後，聽力逐漸進步，其他相關如失眠，肩頸僵硬問題亦獲得改善，因國內期刊目前沒有關於單純使用針灸治療突發性耳聾的病例，故整理此醫案作為針灸治療突發性耳聾的臨床參考。

Idiopathic sudden sensorineural hearing loss (ISSNHL) is a common disease in ENT clinical. The main symptom is sudden hearing impairment. In addition to hearing loss, tinnitus, and vertigo are also seen in these patients. Because the mechanism of sudden hearing-impairment was still not clearly, many theories such as virus infection, blood clots, within the cochlea cellular stress response and immune regulation are discussed. Although the clinical treatments are numerous, the effects are not always clear. Many patients are suffering from the condition persist to this end. This 46-year-old female patient from the beginning of incidence was treated at ENT clinical, and diagnosed idiopathic sudden sensorineural hearing loss using pure tone audiometry. After one month of treatment, the situation has not significantly improved. The patient searched for help at Department of Traditional Chinese Medicine. After acupuncture, the gradual progress of hearing, other related such as insomnia, shoulder and neck stiffness problems have also been improved. Because there is no domestic journals on the use of acupuncture alone in the treatment of ISSNHL. We finishing this medical case as acupuncture treatment of ISSNHL clinical reference.
