針灸在安寧緩和醫療上發展現況與未來願景

摘要

安寧緩和醫療在政府與民間團體大力促成之下已經發展完善，且普遍為病患所使用。時至今日，我國已儼然成為安寧緩和醫療在亞洲的先趨。中醫順應自然以人為本，給予病患身體與情志上的照顧，與安寧緩和醫療理念，讓病患順著病程發展並給予身心靈三方面關懷兩者不謀而合，所以中醫在安寧緩和醫療上也扮演了相當重要的角色。針灸為中醫的一個分支，傳承其天人合一的理念，採用迎隨補瀉手法達到陰陽平衡，因其簡便、有效、快速、安全等特質廣為人知。目前接受安寧緩和醫療患者以癌症患者為大宗，針灸可改善傳統治療導致之副作用，不但療效顯著，沒有藥物引起之副作用。比方說癌因性疲憊、潮熱、憂鬱焦慮情況可藉由針灸獲得緩解；改善失眠、噁心、嘔吐、止痛優於藥物，針灸療效可在療程後持續數周到數月之久。非癌症病患如慢性阻塞性肺病、末期心臟衰竭、末期腎臟衰竭、失智症等接受安寧緩和醫療的需求與日俱增，且已是全球趨勢。針灸包含一系列的療法，未來在安寧緩和醫療這一塊相信有更多更深的發展，並且走得更遠。

Hospice and palliative care are well developed and widely applied among terminal patients in Taiwan with the help of two strong advocates government and organizations, today Taiwan has become the pioneer of hospice and palliative care among Asia. The theory of Traditional Chinese Medicine is conformity with nature and people-oriented, giving patients physical and emotional care coherent to the core value of hospice and palliative care-following natural course of disease, offering physical, mental and spiritual care, thus Traditional Chinese Medicine plays a crucial role in it. Acupuncture emanate from Traditional Chinese Medicine inherent the concept of harmony between man and nature, treating patients as a human, balancing yin and yang by directional supplementation and drainage notable for its utility, fast, safe and reaching remarkable therapeutic effects. Cancer terminal patients are the majority of hospice and palliative care applying acupuncture on them receiving prominent and evident efficacy on relief side effects of conventional therapies without any adverse reaction to drug. After receiving acupuncture treatment cancer-related fatigue, hot flash, anxiety, and depression are relieved, the efficacy of drug to insomnia, nausea, vomiting, and, pain is lower than acupuncture, and the effect of acupuncture treatment can lasting form weeks to months. The needs of non-cancer terminal patients- end stage chronic obstructive pulmonary disease patients, end stage heart failure patients, end stage kidney failure patients, dementia patients to receive hospice and palliative care is surging from pole to pole, acupuncture including a series of treatments can do more and go beyond itself in the near future.
