照顧一位口腔癌病患接受口顎重建手術後之護理經驗

摘要

本篇個案報告是探討一位罹患口腔癌第四期患者接受口顎重建手術後之護理經驗，照護期間自2008年11月20日至12月5日止，筆者藉由觀察、會談、身體評估、查閱病歷等方式收集資料及歸納分析，並運用Gordon十一項功能性健康型態評估，確立個案有急性疼痛、呼吸道清除功能失效及身體心像紊亂等健康問題。護理過程在於針對個案的問題提供個別性與整體性的相關照護，包括減輕術後傷口的疼痛；提供胸腔物理療法，以緩解痰液黏稠不易排出的問題；協助重建正向的自我概念，以改善身體心像紊亂的問題，使個案能獲得最佳的身、心及社會的適應。目的是希望藉由本個案護理經驗，提供日後護理同仁照護相關病人時之參考。

This case report discussed the nursing experience of patient with the fourth stage oral cancer receiving mouth jaw reconstruction surgery. This patient was under my intensive care from 2008/11/20 to 2008/12/05. The data collected by observation, interview, physical assessment and medical record consultation was analyzed following the Gordon 11-item functional health pattern. The problems identified include acute pain, ineffective airway clearance and disturbed body image. The author provided individualized nursing care base on the patient's problems in order to alleviate the patient's pain, to provide physical therapy to improve airway clearance as well as to restore a positive self perception of body image. This project was to make the patient obtain the best physical, mental and social adaptation. The main purpose of this article is to share the experience for caring similar patients.
