摘要

Port-A化療外滲常造成病人組織傷害與住院天數延長。本單位2012年1月至5月共發生6件Port-A化療外滲，發生率高達4.2%，故引發專案動機。分析原因為住院醫師未監督實習醫學生正確完成角針放置、護理師未護理指導，導致病人認知不當。專案目的為降低Port-A化療外滲發生率、提升Port-A角針放置正確率、落實護理指導並提升病人認知。改善方案為：增訂護理指導單張、修改訓練教材、增加醫護人員教育訓練與稽核。專案實施後Port-A化療外滲發生率為0%，實習醫學生均能在住院醫師監督下正確放置Port-A角針，護理師均能落實護理指導，病人對Port-A認知提升至100分。透過本專案使醫護人員均能了解化療外滲發生原因與預防措施，減少對病人的傷害。

Eextravasation of chemotherapy agent from the Port-A catheter often causes tissue damage and prolongs hospital stay. The high incidence rate of Port-A extravasation (4.2%) on the author's ward from January to May 2014 prompted the creation of the current project. It has been noticed the incidence rate of the Port-A extravasation increased when the intern doctors placed the catheter in the absence of a resident or when the nurses didn't provide adequate care instruction to the patients. Therefore, the author revised the content of care instruction for the patients and enforced the training of the medical staff in order to reduce the incidence rate of Port-A extravasation. The results indicated that after with the supervision from the resident, the intern doctors were able to place the port-A catheter correctly; and with the revised patient education and enhanced training, the patients' knowledge to the Port-A extravasation was seen increased. Therefore, the project helped medical staff learn the causes and the preventive actions of Port-A extravasation, which reduced harm to the patients.
