本院為了癌症病人照護品質,建置全癌個案管理照護模式。在業務量激增下,傳統人工篩選個案流程之方式不但費時且一致率低而影響管案品質。本專案旨在透過資訊系統整合以提升腫瘤個案管理師篩選個案流程一致率,分析主因:收案條件定義與系統操作流程不明確、篩選系統沒有良好的分區功能、腫瘤個管師對系統操作教育訓練不足。改善策略:修訂收案定義與資訊系統操作手冊、新增篩選個案之分區功能與辦理教育訓練課程。結果:篩選個案流程一致率由65.3%提升至95%,進而讓篩選個案花費時間縮短,提升腫瘤個案管理師對資訊系統使用的滿意度,不僅增加腫瘤個案管理師工作成效,癌症病人及家屬亦能獲得良好的醫療照護品質。
A rapid increase in cancer patients has rendered traditional procedures for case screening not only time-consuming but also inconsistent, which is detrimental to the quality of case management. Therefore, our hospital established a care model for total cancer case management in order to improve the quality of care. This project involved employing an integrated information system to enhance the consistency of case screening procedures performed by cancer case managers. The aforementioned problem is primarily the result of the inadequate classification function of the current system, ambiguous inclusion criteria and unclear operating procedures, as well as unfamiliarity with the current system's operating interface and inadequate training on how to operate the system. Recommended improvement strategies include formulating clear case inclusion criteria and screening procedures, creating an effective classification function for case screening, implementing training courses, and compiling an operating manual for the information system. The results show that the integrated information system improved the consistency of case screening procedures from 65.3% to 95%, which decreased the screening time from 98.78 to 28.89 min and improved cancer case manager satisfaction with the information system. These improvements not only enhanced the job effectiveness of cancer case managers, but also elevated the quality of health care services provided to cancer patients and their families.
