運用輔助療法於一位舌癌末期個案之照護經驗

此照護經歷為敘述一位育有幼子之舌癌女性個案，因腫瘤復發，壓迫局部組織，面對死亡威脅，導致疼痛、體液容積過量與無望感問題，故引發筆者探討的動機。筆者於2017年11月3日至2017年11月20日藉由Gordon十一項功能性健康型態評估模式，透過觀察、會談、身體評估及病歷查閱等方式收集資料，確立個案有慢性疼痛、體液容積過量、無望感之主要護理問題。筆者運用輔助療法及跨團隊共同照護，提供芳香療法、淋巴按摩、音樂療法等方式，達到減緩疼痛、改善淋巴水腫，促進了個案的整體舒適感，其心靈層面，因擔心無法順利陪伴一歲幼子長大，在醫護團隊的引導下，鼓勵個案、案夫與案母說出心中感受，與完成四道人生，並引導個案說出自己餘生期待，自我價值，讓個案更了解自己想法與自己存在意義，在有限的人生歲月中，用不一樣的方式留下生命的足跡。期望透過實務與文獻結合，提供解決護理問題之相關照護，將護理經驗提供日後照顧此類個案之參考。

The study presents a nursing care experience for a female patient, who was raising a 1-year-old boy, facing the diagnosis of terminal tongue cancer which relapsed within a year. The tumor progressed rapidly and induced severe problems including local tissue compression with swelling and risk of death. From November 3rd to the 20th, 2017, the author assessed the patient with Gordon's 11 functional health patterns and collected data through observation, interviewing, physical evaluation, and medical records. The major nursing-care problems identified were chronic pain, fluid overload, and sense of hopelessness. To help the patient face the suffering and troubles, the author guided her and her family members to express love, apology, gratitude, and say goodbyes to one another in phases. The author also performed complementary therapy by transdisciplinary approach to relieve her of the suffering and improve the quality of life towards the end. This article describes the author's nursing care experience which could provide clinical staffs with more insight into nursing care of similar cases.
