比較在影像導引放射治療中自動影像導引定位驗證系統和錐狀射束電腦斷層的準確性

Comparison of the Setup Accuracy of ExacTrac X-ray System and cone-beam CT in Image-Guided Radiation Therapy

摘要


本研究主要利用假體來測量自動影像導引定位驗證系統(ExacTrac X-ray system)和錐狀射束電腦斷層(cone-beam CT, CBCT)的準確性。首先利用假體表面所做的標記將假體設置於等中心點位置,給予一個已知位移,移動距離選定為0 mm、5 mm、10 mm、15 mm。利用自動影像導引定位驗證系統取得兩組斜位的X光影像,和數位影像重組影像(Digital Reconstructed Radiographys, DRR)進行比對,利用比對結果進行位移偏差修正。之後利用相同方法以CBCT掃描取像,再和DRR進行比對計算位移偏差。所有位移偏差測試皆量測三次,比較自動影像導引定位驗證系統和錐狀射束電腦斷層實際測量值和已知位移的準確性差異。自動影像導引定位驗證系統所測得X軸(左右)、Y軸(頭腳)及Z軸(前後)的平均位移及標準差分別為1.43±0.17、1.14±0.32、2.08±1.16 mm。錐狀射束電腦斷層所測得X軸(左右)、Y軸(頭腳)及Z軸(前後)的平均位移及標準差分別為1.16±0.33、0.42±0.10、0.69±0.21 mm。比較二者結果,在Y、Z軸方向的位移具有統計學上的顯著差異。

The study to compare the setup accuracy measured with ExacTrac X-ray system and cone-beam CT (CBCT) for phantom. Firstly, the phantom was initially positioned on the isocenter using the marks on the surface of the phantom. A known translational shift including X (L-R), Y (S-I), and Z (A-P) axes was performed. The translational shift was selected from (0, 5, 10, 15 mm).The phantom was then imaged with two oblique X-ray images using the ExacTrac X-ray system. These two images were automatically registered with corresponding Digital Reconstructed Radiographys (DRR) to calculate a correctional shift. After this, the phantom was scanned with CBCT, and matched with the planning DRRs using image registration to calculate a correctional shift. The above test was repeated for three times with different translational shifts. The comparison of measured vs. known displacements was used to test the accuracy of ExacTrac X-ray system and CBCT. The translational setup discrepancies in mean±SD with ExacTrac X-ray system were 1.43±0.17, 1.14±0.32 and 2.08±1.16 mm in X (L-R), Y (S-I), and Z (A-P) axes, respectively. And the CBCT were 1.16±0.33, 0.42±0.10 and 0.69±0.26 mm in X (L-R), Y (S-I), and Z (A-P) axes, respectively. Statistically significant difference were found on Y (p=0.001), and Z (p=0.001) axes between ExacTrac X-ray system and CBCT.
