螺旋式頭部與頸椎電腦斷層一次性掃描-短論

Helical Head and Cervical Spine Computed Tomography in the One-time Scan- Short Communication

摘要


急重症的病患,除了頭部之外,頸椎也須納入檢查範圍,因頸椎的創傷與損害常伴隨著不可逆的嚴重後果。目前臨床作法為頭部與頸椎分開掃描,但相對耗費時間長,本文改變傳統的參數設定與掃描模式,利用實際的臨床數據探討螺旋式電腦斷層頭部與頸椎一次性掃描,使用奇異公司16切電腦斷層,蒐集30位急診醫師開立頭部包含頸椎電腦斷層檢查單,記錄掃描張數與時間,由劑量報告表中取得劑量數據,結果得知平均體積電腦斷層劑量指標為39.44±1.92 mGy,劑量長度乘積為1190.26±98.89 cm‧mGy,平均掃描所需時間為12.86±0.63秒。由掃描影像與劑量表得知,螺旋式一次性掃描應用於急診外傷患者為可行之協議,對於難以配合的病患在時間上更有其優勢性,本文提供螺旋式頭頸部電腦斷層一次性掃描協議與臨床實證,供臨床操作者參考之用。

In addition to the head, the cervical spine should also be included in the examination for critical patients, because the cervical spine trauma and damage is often accompanied by irreversible serious consequences. The current clinical practice for the head and cervical spine scan is separate, but relatively time-consuming. We changes the traditional parameter setting and scanning mode, using clinical data to discuss helical head and cervical spine computed tomography in one-time scan. We use GE sixteen slices computed tomography, collecting order of thirty peoples conducted head and cervical spine computed tomography in the one-time Scan, recording the number and scan time, obtaining the dose data from dose report. The results show that average volume computed tomography dose index is 39.44 ± 1.92 mGy, dose length product is 1190.26 ± 98.89 cm‧mGy, average scan time is 12.86 ± 0.63 s. From the images and dose reports, helical one-time scan for emergency trauma patients is the feasible protocol. The patients who are difficult to cooperate with are superior in time. We provide helical head and cervical spine computed tomography in one-time scan protocol to users for clinical reference.
